Third Age Health – Navigating Wellness

The Navigating Wellness Health Checker a is free, quick, confidential and convenient digital tool created to support older adult New Zealander’s health and wellbeing.
You can choose to complete 1 or all 9 assessment topics from Diet & Nutrition, to Hearing & Vision, Cognitive Changes and Mobility. Simply answer a set of yes or no questions, then opt to have the completed summary emailed directly, all in less than 10 minutes. The summary is helpful to have on hand when you’re visiting the GP. Make the most of your appointment or prompt topics for further discussion and support.
There is also a Navigating Wellness Guidebook. This guidebook gives practical advice for older people and their loved ones covering a range of health topics. Each topic includes relevant information, simple tips, resources for further support and interactive self-assessments.
Whaimana – Support My Decisions

Whaimana – Support My Decisions is a website about supported decision-making. Supported decision-making helps people make their own choices about their lives, with the support of the people they choose.
The website has helpful information for:
- – people making choices and decisions (decision-makers)
- – family, whānau, and friends who want to support people
- – paid supporters working for disabled people or older persons with reduced capacity
- – supporters who have may hold legal roles such as EPOA (Enduring Power of Attorney), Welfare Guardian or Property Manager
- – advocates (people who speak up for others).
